Getting Started
Wesley Home Community Health Services can help you stay independent
in your own home. You now have a choice to receive care and services in
your home, and who better to provide those services than a care provider
you trust.
First Step – Free Phone Consultation
Contact Wesley Homes Community Health Services for a free phone consultation by our Registered Nurse. During this meeting our nurse
will assess your individual needs including personal and physical
preferences.
Second Step – Develop
an Individualized Care Plan
Wesley Homes Community Health nurse prepares and reviews a comprehensive
home care plan of care which will include instructions for:
- Personal care: bathing, eating,
dressing, toileting
- Household care: cooking, cleaning,
laundry, shopping
- Health care: medication management,
physicians’ orders
- Emotional care: companionship,
activities, conversation
Third Step – Select Caregiver and
Begin Orientation
Once you and the nurse have agreed on a care plan,
then it will be time to select a caregiver. Our scheduling staff
will come up with the most appropriate caregiver and schedule a
meeting with you. After meeting with the caregiver if you feel comfortable
then services will begin.
Wesley Homes Community Health Services will orient the
caregiver and communicate all information supplied during our assessment.
We will leave a copy of all communication in your home for the caregivers
reference while they are caring for you.
